I, the undersigned, acknowledge that participation in any Perimeter Sports NYC activities, including but not limited to afterschool, camps, clinics, leagues, workouts, and community-based programming—involves physical activity that may carry a risk of injury or accident. I understand and agree with the following: • I certify that I (or my child) am physically fit to participate in athletic activities and have not been advised otherwise by a qualified medical professional. • I knowingly and voluntarily assume all risks associated with participation, including potential injuries such as sprains, fractures, or other medical conditions. • I hereby release, waive, and hold harmless Perimeter Sports Inc, its directors, staff, coaches, volunteers, sponsors, and affiliates from all liability, claims, or actions for damages resulting from injury, loss, or harm to the participant during or as a result of participation in Perimeter Sports Inc events. • I accept full financial responsibility for any medical treatment or emergency services required as a result of participation.

Perimeter Sports Inc often documents programming through photography and video to highlight positive youth development, promote future programs, and celebrate our community. By signing this form, I grant Perimeter Sports Inc and its authorized representatives the right to: • Photograph, film, or record the participant during events and activities. • Use, reproduce, and publish images or videos in promotional materials, including websites, social media, printed materials, and press releases. • Use the participant’s first name, likeness, voice, or statements in connection with the above. I understand that no compensation will be provided for the use of such media, and that these materials will remain the property of Perimeter Sports Inc.
